Everyone Active GP Exercise Referral Scheme

 Who is eligible for the scheme?

  • Individuals must be 16 years and over
  • Classed as inactive by participating in less than 30 minutes of moderate/vigorous physical activity on less than 5 days per week
  • Sufficiently motivated to want to increase their participation in physical activity
  • Have, or be at risk of developing, a long term health condition
  • High Risk conditions can’t be accepted e.g. Instable Angina, unstable or acute heart failure or BMI above 40 (Tier 3)

  • Cancer
  • Obesity

  • Hyperlipidemia

What does the GP Exercise Referral Scheme involve?

Our GP Exercise Referral instructor will find out what is important to your patient and help them to find activities that they will enjoy! A 6 and 12 week review will enable the instructor to support the individual through their first 12 weeks to a healthier lifestyle.

Activities vary at each centre but may include:

  • Walking sports or led walks
  • Group exercise classes such as chair-based exercise
  • Casual swimming or water-based activities such as Aqua
  • Gym based exercise following a program set by our coordinators
  • Social groups including coffee mornings for increased peer support
  • Condition specific activities such as Good Boost technology or Escape Pain for MSK conditions. Find out more about Good Boost here. Find out more about Escape Pain here.

Active Life gym sessions

10am – 12pm = Monday/Wednesday/Thursday/Saturday

6pm – 8pm = Tuesday/Thursday

A L4 GP Referral qualified member of staff oversees these sessions and is on hand to offer any assistance. Consultation appointments will be of similar times but vary due to availability.

Membership options

  • GP Referral DD – The individual will receive a 25% discount for the first 6 months of the membership. There are no upfront fees, and all activities are included (Gym, Swim and Fitness Classes)
  • GP Referral PAYG – This option the individual will pay for each activity they take part in and book. Only the initial consultation and 6 & 12 week review meetings are included free of charge through the scheme. Gym sessions are discounted at £4.40, exercise classes and swim sessions remain at full price.
  • Active Life Swim – The member will need to either take out a GP Referral DD membership or PAYG for each swim session. Initial consultation, 6 & 12 week reviews are free of charge, but please note there is no monitored sessions specifically for the scheme offered at the centre

For PAYG sessions, passes must be purchased at reception or on the app before visiting the gym. Discounts are available for those on any eligible income related benefits, but concessions must be preapproved by our accounts department.

GP Referral Exercise memberships are also available for MSK/Physio referrals, but are subject to an initial consultation with a coordinator to confirm they are happy the patient can exercise safely and without the presence of an instructor. Please note, there are no patient reviews with this membership.
